ASSESSMENT LISTING
In Home & Worksite Analysis
In Home & Worksite Assessments provide practical recommendations for clients to reintegrate themselves effectively in their environment with an emphasis in returning clients to their pre-accident level of function in a safe, cost effective and expeditious manner.
Independent Living Assessments are an objective evaluation of the client's self-care and home environment tasks to determine their ability to function. IADL's are valuable in determining if a client is able to live independently.
In Home Assessments are used to evaluate the various tasks a client may have at home, and to what extent their injury may impair their ability to do so.
Worksite Evaluations provide objective information on the physical demands of the client's job. The results of such evaluations can be used to structure treatment and work simulation programs, as well as to make recommendations on work site modifications or work aides to assist the client in returning to work.

Functional Capacity Evaluation
A Functional Capacity Evaluation (FCE) is a quantitative method of assessing an individual’s measurable work capacity or, alternatively, the level at which the individual is able to function. The FCE matches the individual’s current performance levels to the demands of his/her normal pre-accident.
An FCE is recommended when you want to obtain accurate and quantified information regarding an individual’s work capacity for comparison to specific timed day or workday. The FCE as a tool is able to provide the simulated job stressors and tasks and provide you with accurate information on the individual’s tolerance that the person may have for their actual essential duties of daily living and/or occupation.
An FCE when used in conjunction with a Jobsite Analysis can compare the individual’s present physical abilities to their pre-accident job demands.
The report generated from the assessment will include the full Dictionary of Occupational Titles definition of the position of employment for comparison. This will also provide vocational levels of performance to your staff.

Job Site / Ergonomic Evaluations
Job Site Evaluations provide objective information on the physical demands of the client's job. The results of such evaluations can be used to structure treatment and work simulation programs; make recommendations on work site modifications, or identify the need for aides to assist the client in returning to work. This report may also assist the general practitioner with specifying return-to-work modifications and/or guidelines.

Functional Abilities Evaluations
The Functional Abilities Evaluation is a significant tool that can be used to make objective and reliable assessments of the individual’s condition. These assessments can be especially useful to employers when matching an employee to a position. When the employee’s abilities do not compliment the work environment; it can result in lost time and injuries. An FAE assesses the individual’s, strength, flexibility, balance, coordination, cardiovascular condition, and functional tolerances of lifting, pushing, carrying, pulling and, as well as body mechanics
Fearless Driving Rehabilitation uses the ARCON system, a state-of-the-art system for conducting FAE’s. Using the ARCON system our evaluators perform clinical functional testing, including work simulation tasks.
FAE's provide the referral source with valuable information regarding a client's abilities, limitation, and return to work recommendations. An FAE performed by Fearless Driving Rehabilitation will include completion of the Vernon-Mior, Revised Oswestry questionnaires along with a pain diagram.
We offer four options when requiring an FAE:
- Post Offer Assessment
- Return to Work
- Baseline
- Benchmark
A Post Offer Assessment will help provide valuable information on the fitness level of the employee before starting a new position. The Post Offer Assessment also provides a baseline for possible future injuries to be compared to and confirms that the employee is physically capable of performing the essential duties of the position.
A Return to Work (RTW) evaluation will provide you with a report designed to determine whether an individual is capable of performing the tasks required of him/her as determined by his/her pre-accident or pre-trauma position of employment.
A Baseline evaluation will determine areas of functional weakness prior to initiating a treatment program. This allows the program to be customized to these weaknesses and excludes unnecessary procedures that may not be cost-effective.
A Benchmark evaluation is used in conjunction with the Baseline assessment to determine progress made during treatment.

Return To Work Case Management
Early efforts to help injured employees return to work will increase the probability for successful transition and, thereby, reduce rehabilitation costs Fearless Driving Rehabilitation will assign an Injury Case Manager, Registered Nurse or disability Case Manger with appropriate training who will be accountable for the administration of the case. The Case Manager will liaise with all treatment professionals and keep the client and employer up to date with the case and medical status. The Case Manager will also facilitate hospital discharge where necessary, as well as coordinate, assess and monitor treatment with all associated professionals. This proactive approach for coordinating the activities of management, union, insurance carrier, health care professionals and vocational rehabilitation services will insure a timely and safe return to work.
Fearless Driving Rehabilitation helps to design and implement a comprehensive RTW program including the completion of a Physical Demands Analysis, Functional Abilities Assessment, Vocational Assessment, and Transferable Skills Analysis, for employees returning to work.
At the case level, Fearless Driving Rehabilitation can work with the claimant, insurer and/or employer to develop and coordinate a return to work plan that utilizes graduated or modified duties and job accommodations, where necessary. This benefits both employees and employers as downtime is reduced and the employee’s skills are being utilized during the recovery period. This also, in many cases saves the employer retraining and staff replacement costs.

Neurological Assessments
Our neurologist will conduct a thorough assessment of the integrity of the nervous system and the impact of any neurological condition on the ability to function. Our neurologist may use a variety of tools including a physical assessment of the nervous system and EMG nerve conduction testing. EEG's, brain scans, and MRI's may be recommended to provide objective confirmation of subjective complaints

Physiatry - Physical medicine and rehabilitation
Physiatry is thorough evaluation and diagnosis of musculoskeletal injuries and pain syndromes including soft tissue, fibromyalgia and chronic fatigue syndrome. Our Physiatry spends considerable time interviewing and examining the client to determine if there is any measurable impairment. The report includes recommendations for helping to return the claimant to function as soon as possible.

Psychiatric Assessment
A psychiatrist will provide a detailed and comprehensive evaluation with information on the psychiatric condition of an individual. These evaluations are often used to determine whether or not an injury or illness was responsible for the onset of any determined psychiatric problems

Clinical Neuro-psychology is an applied science with the behavioral expression of brain dysfunction. A Neuro-psychological assessment is an evaluation as to the impact of an injury or illness on an individual's neurological, cognitive and emotional functioning.

Neuro-Psycho-Vocational Assessment
Fearless Driving Rehabilitation can arrange this assessment with one of our consulting Psychologists. The evaluation can determine the degree to which cognitive and behavioral processes have been affected after an Acquired Brain Injury (ABI) has occurred, along with the level of the claimant's intellectual and emotional resources in order to facilitate their rehabilitation.

Vocational Evaluation
A vocational evaluation is a systematic process of identifying appropriate vocational choices, with consideration to the individual's aptitude, abilities and personality traits. Ideally a vocational assessment is performed after or in association with a thorough physical examination (such as a work capacity evaluation), which evaluates the individual in terms of the Physical Abilities Classification System. Also included are services such as Comprehensive Employability Assessments, Transferable Skills Analysis, and Labor Market Surveys.

Fearless Driving Assessment
This is a comprehensive assessment of the evaluee’s driving skills/ Fear after an accident. The evaluee’s is assisted by re inserting them into a driving instructor’s car and retraining them on topics like accident prevention, Winter driving, Skid prevention etc. This assessment and the treatment that follows usually ensure a safer driver with less fear of driving as a result of the vehicle accident.

Catastrophic Injury Management
A Catastrophic Injury is an extremely serious injury that may result in permanent disability, long lasting medical conditions, and a shortened life expectancy. At least one of the body's systems is affected in a catastrophic injury, such as the respiratory system, muscular system, or neurological system. A Catastrophic Injury Case Manager, a Registered Nurse or Disability Specialist with appropriate training and understanding the in Glasgow Coma Scale, will be accountable to the administration of the case/ claim. The Case Manager will liaise with all treatment professionals and keep the client up-to-date with respect to their claim and medical status, facilitate hospital discharge and coordinate, assess and monitor treatment(s) with all associated professionals

Future Care Cost Assessment/ Chronic Pain Assessment
A comprehensive detailed assessment that outlines a disabled person's current and future needs and quantifies the costs related to these needs over the individual's entire life span. This assessment is typically completed for individuals who have experienced catastrophic injury or have chronic health care needs. Future medical and vocational expenses are forecasted with the focus on maximizing independence, minimizing medical complications and, where appropriate, planning for productive work activity. Accommodation, attendant care, transportation, aids to daily living, medical expenses, professional services, education, pharmaceuticals, home modifications, vocational needs and all other associated costs based on the individual's disability are addressed.

Future Costs of Medical / Rehabilitation Benefits Assessment
A specific assessment of a disabled individual's entitlement to med/rehab benefits based on the appropriate legislation.
